Manchester Housing and Redevelopment Authority (MHRA) has provided Congregate Housing Services Programs for the elderly and persons with disabilities since the late 1980s. Widely recognized as a model program, CHSP gives participants the support needed to remain in their MHRA apartments as independent members of the community. CHSP offers services such as meals in a restaurant style setting, housekeeping, health information, personal assistance, transportation, and case management to help alleviate the need for premature placement in nursing homes or other institutional care facilities.
Last year, MHRA's CHSP staff provided 76,435 meals, all prepared and served on site, 4,293 units of housekeeping, 3,752 units of personal care and 910 units of transportation to the 117 participants in five CHSP programs. Below are our five sites:

Supplementing the efforts of the Authority's CHSP staff, our Clinical Services Coordinator oversees MHRA's Elderly Outreach efforts, coordinated by an MSW. The Clinical Services Coordinator, a registered gerontological nurse, also supervises nursing related service provision by the CHSP Licensed Nurses Assistants and coordinates health and wellness education and screening for all residents through our own State licensed home health agency.
The Authority's Elderly Services Department, under which the Congregate Housing Services Program is administered, provides direct services to the elderly and persons with disabilities as well as referrals to other agencies. Staff work with local service providers to assess needs, coordinate services, ensure efficient use of private and public resources, and assist in monitoring and evaluating the delivery, impact, and effectiveness of all supportive services provided.
