Removal Applicaton (HUD-52860) (MS-Word, 423 KB)
Revised 10/2007
This official document (which has received clearance
from OMB) provides instructions for PHAs in completing the electronic application
in PIC. It also contains the Certifications of Compliance that PHAs must attach
as electronic applications to their applications in PIC. In rare instance, SAC
management may also approve a PHA to submit this paper application rather than
make an electronic application through the PIC. However, PHAs must obtain approval
from SAC Management to submit a paper application.
Note: this new version
is currently undergoing additional clarifications and an updated form will be
available soon. Please contact SAC Management with any specific questions about
this form.
Offer of Sale (MS-Word, 16 KB)
In some cases, the development for which demolition/disposition or disposition
activity has been proposed must be offered to the resident organization in place
for that development, or to any group, which says, they represent the resident
s of the development. This Offer of Sale example was prepared to assist applicants
with regard to the Offer of Sale requirement, and should be modified in accordance
with the conditions of each application.
Time Table
(MS-Word, 32 KB)
Applications
must include projected dates for significant milestones in the process, including
the number of days after HUD-approval that the PHA will begin the relocation of
residents and the number of days after HUD-approval that the PHA plans to complete
the relocation of residents. PHAs must also include other significant projected
dates in the Time Table, including the number of days after approval that the
PHA plans to enter into a contract for the proposed demolition and/or disposition,
as well as the number of days after HUD-approval that the PHA plans to complete
the actual demolition or have the disposition closing. Progress in relation to
these projected dates will be monitored by HUD and reporting to Congress. This
Time Table example was prepared to assist applicants with regard to the Time Table
requirement, and should be modified in accordance with the conditions of each
application.
Board Resolution (MS-Word, 12 KB)
All applications must include a Board Resolution in support of the application,
dated after the last resident consultation took place, after the date of the letter(s)
of support from all appropriate local government officials, and after the offer
of sale, if appropriate. This Board resolution example was prepared to assist
applicants with regard to the Board Resolution requirement, and should be modified
in accordance with the conditions of each application.
Local
Government Consultation (MS-Word,25 KB)
All applications must include
evidence that the proposed action was developed in consultation with appropriate
local government officials. PHAs should include a letter from each appropriate
government official with jurisdiction over the affected development indicating
that the local unit of government supports the proposed demolition/disposition
action. If the PHA covers multiple jurisdictions, the PHA must provide a letter
from each local government official with jurisdiction over the affected development.
This Mayor's letter of support was prepared to assist applicants with regard to
the government consultation requirement, and should be modified in accordance
with the conditions of each application.
